General Description:To provide work and career development services for individuals participating in the Pathway to Employment Program. Provision and documentation of job coaching, counseling, case management and employer contact.
Minimum Qualifications: Associate's degree in Health, Human Services or a related field, over three years of work experience, preferably in working with disabled people or training of others, with one year of this experience to include assessment and service plan development or an equivalent amount of education/experience.
Must attend all required Innovations trainings offered by OPWDD. Must complete Agency Driver Certification within 3 months of hire.
